Job description
Caregiver Case Manager
Caregivers Network by Sourcewise: provides case management services, including facilitation and connection with respite care options, and resources to support Santa Clara County family caregivers. The Case Manager assists the caregiver navigate the multifaceted challenges often face while caring for a loved one. Family Caregiver Support services are designed to reduce caregiver burden, enable caregivers to remain in the workforce, and prevent or delay the need for a higher level of care for the care receiver.
Sourcewise Mission: Provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.
Who we serve: Family caregivers are adults 18 years of age or older and caring for an older relative or spouse.
Older care receivers must be age 60 years or older, or individuals of any age with Alzheimer’s disease or a related disorder with neurological and organic brain dysfunction.

Job Type: Full-time position.
Schedule: 8-hour shift. Monday through Friday. 8am – 5pm.
Any schedule changes must be coordinated and approved by the Caregivers Network Supervising Case Manager.
Reporting and Working Relationships:
Report to Supervising Case Manager and work closely with the Caregivers Network Care Managers. Work intermittently with all I&A staff and/or work directly with other agency staff when project appropriate as approved.
Job Type: Full-time
Pay: $27.88 - $28.88 per hour
Expected hours: 40 per week
