Job description

Caregiver Case Manager

Caregivers Network by Sourcewise: provides case management services, including facilitation and connection with respite care options, and resources to support Santa Clara County family caregivers. The Case Manager assists the caregiver navigate the multifaceted challenges often face while caring for a loved one. Family Caregiver Support services are designed to reduce caregiver burden, enable caregivers to remain in the workforce, and prevent or delay the need for a higher level of care for the care receiver.

Sourcewise Mission: Provides adults and their caregivers the tools and services they need to effectively navigate their health and life options. Through a comprehensive network of resources, Sourcewise strives to educate, prepare, support, and advocate for all adults, their families, and their caregivers within Santa Clara County.

Who we serve: Family caregivers are adults 18 years of age or older and caring for an older relative or spouse.

Older care receivers must be age 60 years or older, or individuals of any age with Alzheimer’s disease or a related disorder with neurological and organic brain dysfunction.

Requirements:

  • Accuracy and attention to detail

Duties and Responsibilities

  • The duties below are representative of the range of responsibilities expected of the candidate but are not intended to be all-inclusive:
  • Complete intake for program by phone; conduct phone monitoring & home visits as required by funder
  • Complete assigned paperwork and documentation accurately and on time.
  • Conduct psychosocial assessment and evaluations, including reevaluation.
  • Accurately maintain and update records concerning all consumer information within the Customer Relationship Management (CRM) software.
  • Monitor service delivery and costs incurred for each client.
  • Provide information & referral services to educate and assist caregivers navigate community resource options available by phone, email or in-person.

Core Role

  • Schedule home visits for caregiver assessment based on availability from 9 a.m. – 4 p.m. Monday through Friday.
  • Formulate a customized care plan for every client, including referrals to services at Sourcewise or geographic region the caregiver resides.
  • Determine and coordinate support services to implement the care plan, including internal and external services outside of Sourcewise.
  • Purchase & coordinate client services.
  • Create and execute client care plan.
  • Monitor client costs, and act as client advocate on an ongoing basis.
  • Terminate client in the CRM systems, accurately report closure, and ensure required forms and paperwork are filed promptly.
  • Work collaboratively with Caregivers Network Case Manager to develop support group resources and moderate the group.
  • Facilitate follow-up with client, as required.

Program Administration

  • Report expenditures to Finance team per client and services rendered.
  • Problem-solve discrepancies on invoices by working directly with agencies to correct issues and submit new invoices.
  • Coordinate with Supervising Case Manager on caseload monthly.
  • Use internal databases to produce reports.
  • Participate in staff meetings, workshops, seminars, outreach events, and presentations.
  • Contribute as a facilitator of the Sourcewise Caregiver Support Group as needed.
  • Complete other duties as assigned by Director.

Qualifications

  • Possession of a bachelor’s degree in Social Work preferred or possession of a bachelor’s degree in psychology, counseling, gerontology, or related field.
  • At minimum, two (2) year experience working with adults 18 and/or senior population.
  • Fluent in Spanish, Mandarin/Chinese, Russian, or Vietnamese is a plus but not required.

Job Type: Full-time position.

Schedule: 8-hour shift. Monday through Friday. 8am – 5pm.

Any schedule changes must be coordinated and approved by the Caregivers Network Supervising Case Manager.

Reporting and Working Relationships:

Report to Supervising Case Manager and work closely with the Caregivers Network Care Managers. Work intermittently with all I&A staff and/or work directly with other agency staff when project appropriate as approved.

Job Type: Full-time

Pay: $27.88 - $28.88 per hour
